Myasthenia gravis (MG) is an autoimmune disease characterized by the presence of acetylcholine receptor antibodies in the postsynaptic membrane. Thymectomy is generally used to treat MG but in rare occasions, it could cause MG. We present a case of a 71-year-old patient with aspiration pneumonia accompanied by isolated dysphagia related to MG after thymectomy. The diagnosis of MG was confirmed by electromyography with repetitive stimulation test and detection of acetylcholine receptor antibodies. The patient was prescribed pyridostigmine and oral high-dose steroid. Serial videofluoroscopic swallowing studies were performed on follow-ups, and the dysphagia improved with medical treatment and appropriate compensatory maneuvers.
